Try: Suggest that cr observe his her own cognitive abilities and functions if cr notices that he she is more frequently forgetting how a simple thing needs to be done then he she may be declining cognitively cognitive abilities include remembering something as simple as how to talk write or even walk having frequent short term memory loss when cr was not like that before is also a sign of cognitive decline ask cr if he she notices that he she is not able to concentrate on the tasks that he she is doing or if he she is having a hard time remembering how he she used to do something easily or something that he she does daily but can not remember how to do it now suggest that cr try to observe how he she does things a person with cognitive issues can even forget how to do something even as simple as walking ask cr if he she tends to forget what the names of his her relatives are what things are called or how they got to that place if cr is experiencing these signs he she may be experiencing cognitive decline if cr feels that he she has memory problems suggest that you go with him her on outings just in case cr forgets something or gets lost disoriented you might also suggest that cr carry his her cell phone and write down your number if you or someone else can’t go out with him her
